Request Letters of Accommodation

Request for Letters of Accommodation - Students Registered with Mental Health Disability Services

Students who are registered with Mental Health Disability Services (MHDS) with a temporary disability, or those would like to discuss changes to their accommodations, must contact their Disability Counsellor regarding Letters of Accommodation.

Students who are registered with Mental Health Disability Services (MHDS) with a permanent disability and DO NOT need changes to their accommodations, can complete a  MHDS Request for Letters of Accommodation Form.  Academic accommodations will be approved for the Fall/Winter 2017-2018 academic year when Letters of Accommodation are generated.  If we are unable to process a request for letters from a student registered with MHDS, the student's disability counsellor will be in contact.

Glendon students should contact Counselling & Disability Services, Glendon Site to inquire about their Letters of Accommodation.  Students registered with Learning Disability Services (LDS) or Physical, Sensory & Medical Disability Services (PSMDS) at the Keele campus should submit their requests for Letters of Accommodation to LDS or PSMDS (we cannot generate letters for students registered with another unit within Disability Services).

Requested Letters of Accommodation

Your Letters of Accommodation will be generated and your request for Academic Accommodation will be processed and sent to you via email.
Please permit one week for our office to process your request.
When the Letter of Accommodation , LOA, arrives please be sure to review it and let us know if you have any questions or concerns.

It is expected that students registered with our office who require test/exam, classroom or practicum accommodations will discuss their academic accommodation needs and provide Letters of Accommodations to each of their course directors/professors at the start of the academic term or as early in the term as reasonably possible.
